Cardiovascular disease, kidney disease, and metabolic disease do not merely share common risk factors such as hypertension, diabetes, and dyslipidemia. Accumulating evidence points to a cardiovascular-kidney-metabolic (CKM) interconnection, in which kidney diseases such as chronic kidney disease (CKD), cardiovascular diseases such as heart failure and myocardial infarction, and metabolic abnormalities such as insulin resistance and visceral fat accumulation mutually promote one another’s progression. In addition, the recent addition of serum creatinine to Japan’s legally mandated health checkup items is expanding the foundation for early detection in the field of kidney disease. Against this backdrop, there is a growing need to take a cross-cutting look — from a perspective common to cardiovascular, kidney, and metabolic disease — at how to better connect health checkups with subsequent medical examination and treatment. At the same time, international discussion is advancing on how to detect and evaluate risks, such as genetically driven cardiovascular risk, that are difficult to capture through existing checkup items. This points to the need to clarify the key issues facing Japan’s health checkup system and to discuss how it might be improved. Given this background, concrete consideration — from a perspective that spans the cardiovascular, kidney, and metabolic fields — is needed on how to build an effective mechanism for connecting health checkups to subsequent treatment.

In response, HGPI hosted the Multidisciplinary Symposium for Cardiology, Kidney, and Metabolic Medicine. The symposium first organized the current state of Japan’s health checkup system and the challenges surrounding post-checkup treatment, then focused on the ideal form of data infrastructure — including the linkage of health checkup and clinical data — as the axis for discussion. Patients and people living with these conditions, policymakers, experts, insurers, and government officials came together to discuss measures for connecting checkups to treatment and ongoing management, and to look ahead to future directions.
